The Formerly Incarcerated Transition (FIT) Program connects formerly incarcerated individuals with chronic disease, mental illness, and/or substance use disorders to healthcare services. Partners with CCT, UNC Family Medicine, NC DPS, and Mecklenburg County Re-Entry Services.

Hours of Operation
Monday-Friday 8:00am-5:00pm with lunch break 12:00pm-1:00pm daily, Dental Clinic hours are 8:30am-4:30pm Monday-Thursday and 8:30am-2:00pm Friday
